Reason:
• All students entering into kindergarten are required to have a 2 nd dose of Varicella vaccine.
• All students entering into 6 th grade and 10 th grade are required to have a Tdap vaccine before beginning the 2007-2008 school year.
This is state mandated!!!
Each vaccine will cost $14.00
No child will be turned away for inability to pay
YOU MUST BRING IN A SHOT RECORD
In case you cannot make this clinic, you could go to the Tri County Health Department at 10190 Bannock St, Suite 100.
Tuesday mornings from 08:30AM to 10:00AM
Appointment line: 303-451-0123.
The shots will be the same cost as above.

La Razón:
• Todos los estudiantes que entran al kinder requieren a tener una segunda dosis de la vacuna de Varicela.
• Todos los niños del grado 6 y 10 son requeridos a tener un refuerzo de Tdap (Tetano, Difteria y Tos Ferina) para entrar a la escuela antes de empezar el año escolar 2007/2008.

Ningún niño será rechazado por inhabilidad de pago.
La cartilla de vacunas es requerida para recibir vacunas.

NEWS FROM THE HEALTH OFFICE
The State Health Department has increased the requirements for immunizations to begin 2007-2008 school years. The requirements have changed for Kindergarten, 6 th and 10 th grade students
KINDERGARTEN STUDENTS:
Colorado will now be requiring a second dose of varicella vaccine (chickenpox) beginning at kindergarten entry. The complete list of vaccines required for kindergarten entry is as follows:
DTP (Diphtheria, Tetanus, Petussis) 5 Doses upon school entry
Unless 4 th dose was given on or after 4 th birthday. In that case, only 4 doses are required.
POLIO 4 Doses upon school entry
Unless 3 rd dose was given on or after 4 th birthday. In that case, only 3 doses are required. Written evidence of a laboratory test showing immunity eliminates the need for this immunization.
MMR (Measles, Mumps, Rubella) 2 Doses upon school entry
The first dose must have been given on or after the first birthday. Written evidence of a laboratory test showing immunity eliminates the need for this immunization.
HEPATITIS B 3 Doses upon school entry
Written evidence of a laboratory test showing immunity eliminates the need for this immunization.
VARICELLA (chicken pox) 2 Doses upon school entry
Must have been given on or after the first birthday. Written history/evidence of the disease by a health care provider eliminates the need for this immunization.
6 TH AND 10 TH GRADE STUDENTS:
Tetanus/Diphtheria/Pertussis (Tdap)
Pertussis or whooping cough can be a very serious disease in infants and small children. It has been found that frequently, siblings who have Pertussis disease can expose other children in the household. Colorado ranks 5 th of all states in the country with this disease. In Colorado , Tdap vaccine will be required for all 6 th and 10 th graders this upcoming school year (2007-2008) and in subsequent years that schedule will incorporate other grades.
It is the responsibility of the Parent/Legal guardian to present proof of immunization at the time of registration.

Same requirements as Kindergarten students.
2ND, 3RD, AND 4TH GRADE STUDENTS:
DPT= 4 doses
POLIO= 3 doses
MMR= 1 dose
HEPATITUS B= 3 doses
5TH AND 6TH GRADE STUDENTS:
DPT= 4 doses
POLIO= 3 doses
MMR= 1 dose
